1. If you do not know your 學校編號:
Your school code is the 6 digit EMB Registration number given to your school, please ask your principal.

2. If you do not know your log in ID:
The following is an explanation what you log in ID is
- 學校登入賬號(九位字數)例如: p123456wd 或 s654321am
- 上述行頭字母P字解作小學, 或S字解作中學
- 第二至第七位字數要輸入教統局給予 貴學校的六位數字編號
- 第八至第九位字數請輸"am"代表上午校,"pm"代表下午校或"wd"代表全日制
- 如學校有小學部及中學部, 請分開兩個登記, 一個是登記小學, 另一個是登記中學
- 如有任何有關登記之困難, 請用電郵與本會聯絡, 電郵地址: spfas@childhealthhongkong.com . 程式保養工作人員是兼職職員, 他們不能于電話解答問題 , 多謝合作。

3. If you have forgotten your password, but remember the email address you used to register into the scheme:
A. Go to www.childhealthhongkong.com/spfas/registration20062007 , click 1 , to check that you have actually registered this year. If your school name appears on the list then
B. Go to http://www.childhealthhongkong.com/school_system/2006_7/login.php
C. type in your log in ID, see 2. above
D. click "forgot password"
E. then in next page, please type your login ID again
F. then click the Register button.
Your password will be sent to your registered e-mail address automatically.

4. If you have forgotten your password and forgotten the email address you used to register into the scheme:
For security reasons, your school principal must write to Hong Kong Childhealth Foundation by fax to 28863166 providing us with the information enclosed in the draft letter enclosed.
